No TADs to Achieve Bilateral Intrusive Force

Patient Information

Male, adolescent

Permanent dentition

4D Aligner™

Treatment duration: 3 months

Treatment Story

The patient’s tooth #26 had supra-erupted, causing insufficient occlusal clearance for the crown restoration of tooth #36. The treatment plan involved opening clearance for the crown restoration by distalizing tooth #27 in two trays and intruding tooth #26 in one tray.

The aligners were designed with direct printed buttons and subgingival trimlines for elastics, which achieves bilateral intrusive force with no temporary anchorage devices (TADs).

After 3 trays in 3 months with no pain, patient was ready for his implant procedure.

No Attachments to Expand Arch, Solid Tongue Spikes to Correct Tongue Habit

Patient Information

Female

Mixed dentition

4D Aligner™

Treatment duration: 2 months

Treatment Story

The patient had a bad tongue thrusting habit, leading to  the anterior protrusion and a narrow arch. The treatment plan involved tongue habit correction by using togue spikes and expanding arch.

The aligners were designed with no attachments, multi-thickness and  solid tongue spikes.

The patient was adapting well to 4D Aligner™ and tongue spikes. Initial improvements in expanding and tongue positioning are evident.

The treatment is still in progress.

No Attachment to Solve Lower Incisor Root Torque Issue

Patient Information

Female, adolescent

Permanent dentition

4D Aligner™

Treatment duration: 7 months

Treatment Story

The patient had a lower incisor positioned too far facially, with a Class I malocclusion and torque issue. The treatment plan was to use 4D Aligner™ to move the incisor root to a healthier position.

The aligners were designed with no attachments, multi-thickness. Additional thickness was applied at the neck of anterior teeth, with pressure on the gingival 1/3 buccal and incisal 1/3 lingual areas to correct the root.

After 7 months' treatment, the root torque issue was solved without using any attachments.

No Attachments to Correct Deep Bite and Achieve Premolar Rotation

Patient Information

Female, adolescent

Permanent dentition

4D Aligner™

Treatment duration: 12 months

Treatment Story

The patient had worn front teeth and was diagnosed as a Class II malocclusion with deep bite.

The treatment plan involved creating overjet and anterior open bite to prepare for restoration of worn dentition. The space in lower anterior was maintained to create reduced overjet.

The aligners were designed with no attachments and multi-thickness.

The patient responded well to the aligner treatment. Notably, despite the absence of attachments, the patient's upper first premolars showed significant rotation, and there was a marked improvement in the deep overbite.

The treatment is still in progress.

*The case is completed by Bill Layman DMD, MBA, Straighten Up Orthodontics.
